Welcome to The Daily School Route (DSR) Survey

Dear Participant,

This year, your school is joining the Daily School Route (DSR) initiative, an active transportation system for kids. The aspiration of the DSR is for 100% of students who live close enough, to walk/wheel to and from school daily. Walking/wheeling to school lets students enjoy the benefits of independence, improved physical/mental health, and a healthy environment.

The purpose of this survey is to identify the ways that students get to school (walk, bike, bus, drive, etc.), the routes they take when they walk, and the changes that are needed to make it easier for students to walk/wheel to school. This will be used to inform transportation planning at your school. Please take a few moments to complete this survey.

Privacy: To protect your privacy, this survey will not require you to share your name. Any information that you do share will remain anonymous. The results will be reported in aggregate. Additionally, please note that we will not reuse, share or sell your personal information.
